Position: IV Therapist

Service Provider Questionnaire

Hello there! Thank you for your interest in the IV Therapist contractor position with CNM Health & Wellness IV.

Please note the following:

This is 100% mobile contract work to serve our customers in their home, office or hotel rooms, and requires the therapist to be willing and able to drive to:

- Miami (Miami Beach, Downtown, Brickell) and surrounding areas

- Fort Lauderdale (Las Olas and surrounding areas)

- Delray Beach / Boca Raton (Downtown Atlantic Ave & Mizner Park and surrounding areas). 

Travel within 20-25 miles from your area.

- Have valid and current drivers license and car insurance

- Have a valid and good standing professional certification / license

-  A requirement of all our IV Therapists is to have successfully placed at least 1,500 PIVs.

- An expectation of all our nurses is that each nurse must maintain a minimum of a 95%+ success (first stick) for all our clients.

- A requirement for this position - all nurses must have active NSO Policy. Info on NSO Policies can be found at nso.org
